Douglas Thomas Hoffman, Second Lieutenant, U.S. Army, Field Artillery
Thrown from a horse on May 18, 1919, while stationed at Camp Logan, Tex., and died May 23 from injuries received. He is buried at the Ottawa Avenue Cemetery in Ottawa, Illinois.
Survived, at the time of his death by:
Parents, Dr. and Mrs. J. R. Hoffman, 1231 Greenwood Av., Wilmette.
Born June 9, 1893 in Ottawa. Received his education at New Trier High School, Northwestern University and the University of Michigan.
Entered the service September 20, 1917 at Camp Grant; went to Officer's Training School at Camp Taylor, where he was commissioned second lieutenant and was then sent to Camp Logan, Tex.
